Equipment Resupply Forms There are two different ways to request a refill on equipment. Select one of the two options below: Digital document that you can email (DOC) The digital document is a resupply form in Microsoft Word format that you can download, open in Microsoft Word, fill out on your computer, and then email back to IOWATER staff. Full instructions, including email instructions, are provided in the document. Hard copy document that you can send in the mail (PDF) The hard copy document is a resupply form in Adobe PDF format that you can open in Adobe Reader, print out from your computer, fill out by hand, and then mail to IOWATER staff.
